Change of Name or Address

Pursuant to Section 28 (4) of the Teachers Registration and Standards Act 2004 a registered teacher must notify the Registrar within 28 days of any change of name or address. Maximum penalty $1250, Expiation fee $160.

Change of Name details must be provided in writing with a certified copy of your marriage certificate, decree nisi, or deed poll by post or in person to Level 6, 70 Pirie Street Adelaide 5000.
Faxed copies will not be accepted.
